Thank you for choosing Petoskey Ear, Nose and Throat Specialists, Northern Michigan’s Premiere ENT practice! Our physicians and staff understand that your time is valuable. In order to make your first visit with us as worthwhile as possible, we ask that you prepare ahead of time by filling out our patient forms and arriving at least 20 minutes prior to your appointment time. Please bring your photo ID and all insurance cards to every appointment so we can ensure that your policy is active and will cover your visit. Copays are due at when you check in for your appointment. Patients without insurance are required to pay in full at the time of service.
If you prefer to use email or if your appointment is scheduled soon so there wouldn’t be sufficient time to mail out your paperwork, we can email all forms in pdf format to you. These must be hand carried to the office; email is not secure and completed forms should never be emailed. Starting in August 2014, we can email you an invitation to register on our new Patient Portal where you can enter your demographics (address, date of birth, phone numbers, insurance information, etc.) and download forms to fill out for your medical history.
